Job Description:
Job Responsibilities:Position Summary: This position serves as
the Targeted Case Management (TCM) and Community Support program
(CSP) Advanced Practice Nurse Practitioner and is responsible for
psychiatric care of TCM and CSP participants; working as a
colleague with the program Psychiatrist. Essential Functions:1.
Provides ongoing psychiatric evaluations of clients after initial
evaluation by the program Psychiatrist2. Prescribes medications and
provides ongoing assertive medication management to clients.
Pragmatically evaluates options and decides upon treatment course
that will optimize safety and community success within situational
limits. 3. Routine staffing with the Psychiatrist, nurses, case
managers and other members of the CSP/TCM treatment team,
incorporates staff input regarding observations of client's
behaviors and capabilities while prescribing.4. Conducts clinical
consultations on an as needed basis and participates in planned
interventions.5. Participates in treatment planning for consumers
as needed.6. Documents or dictates results of evaluations in a
timely manner7. Orders lab work, testing or other procedures as
neededOther Duties and Responsibilities: 1. Other job-related

position Job Qualifications:Minimal Qualifications: Possess Chapter
N 8 Certification to practice as an Advanced Practice Nurse
Prescriber and registration as a Registered Nurse; completion
continuing education training as required by the board of nursing;
no convictions or license revocations that would fail a caregiver
background check; sufficient malpractice insurance to meet agency
requirements. Knowledge, Skills and Abilities: Good clinical skills
especially related to medical issues, mental illnesses, substance
abuse disorders and Evidence Based Practices; knowledge of DSM V,
psychotropic medications and their side effects; ability to
communicate effectively and in a positive manner with staff and
clients; ability to respond appropriately in crisis situations;
clients' rights including principles of ethics and confidentiality;
good organizational and dictation or typing skills; effective use
of time and resources to help maintain quality services at the
clinic;pm21 Other Job Information (if applicable):Work Relationship
and Scope: Reports directly to assigned program director; works
closely with the CSP/TCM Psychiatrist, nurses, case managers,
clinical coordinators and clients; has telephone contact with other
medical persons, clinics and hospitals located in the community;

funding sources; conduct self in an ethical manner; maintain
professional and respectful relationships with program staff, other
WCS staff, clients and all external persons and agencies involved

mission.Working Conditions: Work is performed in the clinic;
effective use of time and resources to help maintain quality
services at the clinic; clients may be unstable and symptomatic at
times; hours average at least 8 hours per week. Physical Demands:
Position is mobile within the clinic setting and with some time in
the community; most of the work is done sitting at a desk while
