Care Physician (PCP) in our organization is expected to: Take
responsibility for outcomes: The PCP is accountable for delivering
strong clinical care and cost-effective outcomes for a panel of up
to 450 patients. They recognize the significant impact they can
have on patient outcomes by fostering trusting relationships and
encouraging behavior changes. Act as a health coach: Rather than
merely consulting on illness, the PCP helps patients establish both
short-term and long-term health objectives, collaborates with them
to achieve these goals, and regularly follows up to ensure progress
toward improved health. Simplify actions: The PCP prioritizes tasks
effectively to make behavior changes more achievable for patients
and guides their care teams toward performance goals. As a
value-driven, outcomes-focused organization, we regularly assess
the following metrics for each PCP's patient panel to support their
success and advancement to partnership status: patient admissions
per thousand, utilization of 18-21 appointment slots daily (with
new patients counting as 2 slots and follow-ups as 1), CGCAHPS
(patient experience), closure of clinical gaps, and medical cost
measures. Each PCP will have specific targets for these metrics and
will collaborate with center and market leadership, as well as care
teams, to meet these goals. A positive culture is vital in our
medical centers, and as leaders, PCPs are expected to promote a
culture of love, accountability, and passion alongside center
leadership. The PCP must demonstrate the ability to work both
independently and collaboratively with other healthcare
professionals. They will closely coordinate with relevant managers
and medical directors to ensure adherence to guidelines and
participate in risk and quality management programs, clinical
meetings, and other necessary gatherings that support patient
health and organizational objectives. The PCP will comply with
departmental goals, performance standards, regulatory requirements,
quality patient care standards, and established policies and
procedures. KEY JOB DUTIES AND RESPONSIBILITIES: Independently
evaluates both acute and non-acute clinical issues. Conducts and
documents physical assessments and patient histories, analyzes
patient condition trends, and formulates, documents, and implements
management plans based on collected data. This includes assisting
in developing nursing care plans and providing appropriate
counseling and education to patients, families, and significant
others. Plans patient care utilizing comprehensive knowledge of the
specific patient population and/or protocols, anticipating and
identifying common physiological and psychological issues while
considering the patient's cultural background, understanding level,
personality, and support systems. Acts as an advocate for patients.
Manages patient care, which includes: Writing orders for
admissions, transfers, and discharges. Ordering and interpreting
necessary laboratory and diagnostic tests. Prescribing appropriate
medications and treatments. Referring patients for consultations as
needed (e.g., dermatology, neurology, ophthalmology, etc.).
Documenting through detailed progress notes and summaries.
Performing invasive procedures independently upon demonstrating
documented competency. Participating in patient care rounds and
conferences, communicating management strategies to the care team,
and collaborating with multidisciplinary team members to ensure
effective patient management. Identifying situations requiring
immediate physician attention and initiating life-saving procedures
when necessary. Utilizing advanced communication skills to resolve
complex issues and enhance processes and services for patients.
Additional Responsibilities may include: Collaborating with
multidisciplinary team members to assess and improve current
healthcare delivery systems and implement new practice patterns as
appropriate. Engaging in external activities that promote personal
and professional development. Coordinating arrangements and writing
orders for skilled nursing facility (SNF) discharges, completing
necessary paperwork. Working with physicians, nurses, physical
therapists, social workers, families, and key caregivers to
transition patients to lower levels of care when medically
appropriate. Introducing themselves to patients and families,
explaining the nurse practitioner's role. Facilitating
patient/family conferences to review treatment goals, optimize
resource use, provide education, and identify post-SNF needs.
Building collaborative relationships to empower patients and
families in making informed decisions regarding care goals,
palliative care, and hospice options. Facilitating discharges to
appropriate care levels and utilizing preferred providers for
additional services as needed. Prescribing medications based on
state regulations. Performing other duties as assigned and modified
at the manager's discretion. KNOWLEDGE, SKILLS, AND ABILITIES:
Proven track record of consistently meeting clinical performance
metrics in the current role. Strong critical thinking and
problem-solving abilities. Excellent communication and
interpersonal skills. Effective time management skills, with the
ability to perform well under pressure. Must exhibit compassion and
empathy, along with strong listening skills. Detail-oriented, with
a keen focus on patient charts, medications, and follow-up care
details. Basic computer proficiency and familiarity with Microsoft
Office Suite. EDUCATION AND EXPERIENCE REQUIREMENTS: Advanced
Nursing Practice (ARNP) or a similar advanced degree in Nursing is
required. Current Nurse Practitioner Certification in the relevant
state is required. Board certification by AANP or ANCC is preferred
but may be mandatory in certain states. A current DEA number for
prescribing controlled substances may be required based on state
regulations. A minimum of one year of clinical experience is
preferred, though strong new graduates will be considered. Basic
Life Support (BLS) certification from the American Heart
Association or American Red Cross is required within the first 90
days of employment. SALARY RANGE: $214,700 - $306,714 EMPLOYEE
